One entered ready up mode, so is ignored by the duplicate lobby detection.

I've seen this happened too. Could it have something to do with a lobby that ready's up, then opens up again? There could be a time frame where the lobby is ready, so someone makes an identical lobby, but the first one doesn't actually start.

have nothing to do with it  u can probably open 5 or more badwaters.

Can we make it  just one map  eu hl  no matter if mumble or not ? anything else would be useless.

Yeah this system is definately not working, I forgot to screenshot it but there were 3 EU badwater HL lobbies up at once.
