I pre-readied up during a lobby and once all slots were filled, it carried on to counting down as if the lobby wasn't readying up.

I got no pop-up to ready and eventually got kicked from the slot

This looks like a refresh-related bug.

Some players see better refreshes by turning off ad-block and possibly adding "https://" to the URL and not "http://"

 

Admittedly, sometimes tf2c starts lagging out a bit which can cause similar issues, although 17/18 people in your image seem to have got the event.
